HIPAA

The Health Insurance Portability & Accountability Act (HIPAA) requires that the University train all workforce members about the University HIPAA Policies and those specific HIPAA required procedures that may affect the work you do for the University.  Workforce members include all UCSD Health Sciences salaried employees and non-salaried personnel, voluntary physicians with staff privileges, all physicians and associate staff members with medical staff privileges, volunteers, registry personnel and temporary personnel and other health professions students and trainees.

 

HEALTH INFORMATION PRIVACY AND SECURITY
SELF TRAINING MODULES

(Mandatory Training)

After you have completed the appropriate training module(s), please print out and complete the acknowledgement of training certificate(s) and give it to your department supervisor/leader as proof of training.  To view the modules that are in .pdf format, Adobe Acrobat Reader version 5 (or later) is required.

The UCSD Medical Center (UCSDMC) "Notice of Patient Privacy" booklets (form #151-020) and the "Acknowledgement of Receipt of the Privacy Notice" form (form #151-035) can be ordered online through the UCSDMC Forms Management web site http://forms.ucsd.edu.
